It would seem the old E111 form for getting free treatment in other EU countries has now been superceded by a European Health Insurance Card

I found this link on Capn Barefoot forum, tried the on line application myself and was pleasantly surprised by the speed of the response

Its a bit like taking a first aid kit with you, lets hope it's never used

I've been carrying mine around for years and never even gave it a thought that there might be an update. They want your NI number, so I'll do it tomrrow.

yes you need your NI or NHS number to apply

If you applied for an E111 in the last year or so they send you the card automatically - I got mine the other day.
